Dr Alex Craven is a general surgeon specialising in Obesity and Upper Gastrointestinal surgery. He provides consultation and surgery through Advanced Surgical and Darebin Weight Loss Surgery and also works as an Obesity and Upper Gastrointestinal Surgeon at the Austin Hospital.

Alex studied medicine at the University of Queensland and trained in general surgery at the Austin and Northern Hospitals through the Royal Australian College of Surgeons (RACS), before completing post-fellowship training in obesity surgery and upper gastrointestinal surgery.

Dr Alex Craven is most passionate about working with patients to control obesity and its related diseases, in order to live longer, healthier lives. Society’s expectations of what a ‘healthy’ body looks like don’t always align with the science. Individual variation in body size, shape and color are normal, and part of what makes us human.
The harm caused by overweight and obesity doesn’t always line up with how high people become. Likewise, returning to good health doesn’t always require getting to a certain size. In fact, a lot of harm can be done by pushing too hard for weight loss at any cost.
After bariatric surgery, people get healthier, live longer, function better and improve their quality of life. Those  are really good reasons to have an operation! And yes, they get smaller and lighter too, but let’s not confuse that with the real goal.

I want you to exercise! All of you! Exercise saves and improves lives. It’s just often overrated as a standalone weight loss or obesity treatment.
Body weight is influenced by a whole host of factors, exercise is one, but far from the strongest. And it’s not as simple as exercise meaning “more calories out” because the body compensates between bouts of exercise to keep overall energy use under control.
There are really good ways to measure the effect of exercise:
1️⃣ Are you doing it? (Over the long term)
2️⃣ Are you meeting the general activity guidelines (at least 2.5hrs of moderate intensity aerobic exercise and 2 sessions of resistance training)
3️⃣ Progress over time - can you go father, or faster, or is going the same pace for the same time easier now? Can you lift heavier, more reps or more often?
Weight loss is not a good way to judge the success of exercise, and in particular if we do so, we may get discouraged when weight inevitably plateaus, even though the exercise was never to blame.
So make sure you and your team know what exercise is good for (and not so good for) and do it for the right reasons!

Please comment on what matters more than weight for your health.
Good medicine prevents death, controls or cures disease and improves health and quality of life. 
With all of these things, there are much better ways to set goals and judge success than body weight or BMI. 
For the people I meet there are also things that are of particular value to them as individuals we should focus on.
Make sure you and the people you love know what they are, and if not ask your treating team about EOSS, cardiovascular risk factors, and the best indicators for control of the diseases or symptoms relevant to you (spoiler alert - the percentage of weight lost will be more useful than current body weight and BMI and a lot of other factors will be more important than measurements).

I’m talking about Type 2 diabetes- a common disease that responds very well to effective weight loss or obesity treatment. In particular metabolic or bariatric surgery offers a good chance of complete and sustained remission (in other words it goes away - no medication, normal sugars!)
Like most diseases, the best chance of cure comes:
1️⃣ If we strike early (for instance within 5 years of diagnosis, when medication requirements are low and while the pancreas is still working fairly well)
2️⃣ If we take it seriously (stepping up steadily from diet and lifestyle to medications to surgery as soon as we know one isn’t working, or going straight to surgery) 
Far too often we see people who instead have spent years, if not decades, with less effective treatment and are suffering the consequences. That’s not OK - anyone diagnosed with Type 2 Diabetes (or any weight related disease) deserves the truth about their options!

It’s stretching the analogy, but often people with end up with significant nutritional issues after cancer surgery, and less commonly after bariatric surgery. It’s always a lot harder to fix the issue after the fact than prepare well and prevent it (or at least minimise its impact).
More recently we’re seeing a lot in the media about overdosing on potentially toxic ingredients in supplements and multivitamins- another issue dieticians will help you avoid. So if you have had cancer or bariatric surgery, check with a dietician before starting or stopping supplements or multivitamins too.

Of course I’m talking about type 2 diabetes, the most common form and a leading cause of death in Australia. The chance of achieving remission is highest with surgery performed sooner after diagnosis, while medication load is lower and some blood tests can help us calculate the chance of achieving remission. 
Even if remission is not achieved, control of diabetes is greatly improved and there is good evidence for surgery greatly decreasing the likelihood of someone dying of diabetes related causes.
People have a right to know ALL their options and chose for themselves based on information, not myths. Unfortunately we still see long delays between diabetes diagnosis and surgery being discussed as an option. It’s not for everyone, it’s not the easy way out, nor is it dangerous (in fact it’s the safest way to go long term) but it is an option everyone diagnosed with type 2 diabetes and obesity should be aware of.
